To objectively record and help you improve your health every day, Hio applies the EQ-5D-5L assessment method, providing overall care results as well as the best health improvement direction with you throughout the care process. 

What is the care pathway?

At Hio, each care recipient (NCCS) is considered an independent individual and is provided with a comprehensive care plan, including personal activities, nutrition, and daily exercise. This roadmap helps NCCS stay healthy, families feel secure, and the care team perform at their best.

How to get a Care Pathway (LTCS)

LTCS is provided free of charge to customers who register for a schedule of 1 month or more. You only need to provide detailed information according to the questionnaire, the more complete the information, the more suitable and optimal the itinerary will be.

How will the information provided be shared?

Hio is committed to protecting information according to the privacy policy. Data is only used internally to build LTCS, serving NCCS, family members and Hio employees.
